Your first visit with us will establish a vital foundation for our relationship
with you. Unlike many other dental offices, we give you time to get to know your
dentist. In fact, most new patients leave our office saying that they have never
received so much individual care before!

You’ll find all the practical information you need, such as your New Patient Forms,
a map and directions to our office, practice hours, and payment procedures.
What to expect at your first appointment :
You will first meet with Dr. Sayeg in a private setting so that he can answer any
questions you may have and to discuss your needs and wants for your dental care.
Then we will gather important information including your medical history, past dental
care, current dental health and address any problems you are concerned about. If
you wish, we will also schedule time with our hygienist for your dental cleaning
at the same time.
Being well-prepared for your appointment will ensure that the doctor has all of
the needed information to provide the best possible care for you. It will also help
relieve any unnecessary anxiety you may be feeling. So take a few minutes and educate
yourself on your symptoms by reviewing the content on this website.
Unlike many other offices, we respect your time. You will be seen at your appointed
time and we make every effort to keep your visits to a minimum. So, please arrive
on time so that we are able to give you our full attention and complete your treatment
without inconveniencing patients scheduled after you.
Your first appointment with us usually lasts one to two hours depending on your
needs.
Financial Information :
When you schedule your first appointment, we will be happy to give you an estimate
of your charges.
If you have dental insurance, as a service, we will accept the assignment of your
insurance benefits directly to our office. We do not currently participate in any
PPO or DMO plans, however we will file to any insurance plan where you may go to
the dentist of your choice or have “out-of-network” benefits.(See

If you would like us to confirm your benefits, we will be happy to call and check
for you in advance.
We do ask that you pay for treatment on the day it is performed. For our patients
with dental insurance, this would include your deductible (if it applies), and any
other amount not covered by your plan. Please be aware that your estimated portion
will be due on the day of treatment, and we can never guarantee an exact amount

For your convenience, we accept cash, checks, all major credit cards and CareCredit.
Our office staff is always happy to discuss financial arrangements prior to initiating
treatment. If you should need extensive dental treatment, we do offer several payment
plans, including financing and no-interest options with CareCredit.
Patient Forms
You can save some time at your first visit by printing out and completing the Welcome
and Heath History forms in advance of your appointment. Just fax them to us at 404-256-9661
or bring them with you to your appointment. Thanks!
Additional Practice Information
Appointments :
We understand your time is valuable and will do our best to see you at your appointment
time. Since we do not over book our schedule, we promise that during your visit
we will give you 100% of our attention while being attentive to your needs and concerns.
Your scheduled appointment time has been reserved specifically for you. Therefore
we request your commitment to keeping your appointments and arriving at your appointed
time. If you should need to change or cancel an appointment, we require 24-hours
notice to avoid administrative charges to your account.
Dental Insurance Information
:
Our practice is committed to providing the highest quality and conservative dental
care in a safe environment. This commitment involves many factors including skill
and level of care from our team, quality of materials used, quality of equipment
used, and level of continuing dental education of our doctor and team.
Insurance companies set arbitrary reimbursement fees for specific dental procedures.
These reimbursements have no consideration for the level of care that Dr. Sayeg
provides. Dr. Sayeg is able to provide the finest care available because he is not
affiliated with any of these dental insurance contracts (i.e.: PPO, DMO plans).
Most services performed in our office are covered by most insurance plans. The rate
at which they are covered usually can be determined prior to the initiation of any
treatment. Our treatment coordinators will do everything possible to assist you
in obtaining maximum benefits through your contract. Please keep in mind that you
are responsible for your total obligation should your insurance benefits result
in less coverage than anticipated. As a service to our patients, we will gladly
process your insurance claims and estimate your deductible and portion not covered
by your plan. We do ask that you pay the estimated amount not covered by your insurance
at the time of treatment.
When payment from your insurance company is received, any estimated amounts not
paid by your plan would be your responsibility. If your insurance pays more than
what we estimated, that amount would be refunded to you.

Payment Plan Information :
We will be happy to work with you in planning your treatment to fit your budget.
Depending on the type and amount of your treatment, payment plan options may be
available.
Those options may include :
- Dividing your cost between several appointments.
- Predated payments up to three months.
- No-interest payment plans for 6 or 12 months through CareCredit.
- Financing up to five years with CareCredit with an interest rate of 11.9%
- 5% courtesy for treatment over $1000.00 if paid in advance
