They have become increasingly popular over the past 10 years, and thousands have been placed to strengthen broken down teeth, mask discoloration, brighten teeth, and to improve a smile. How are they different from other crowns? Traditional crowns have a shell of ceramic that is fused to a high noble metal (usually gold). The result is generally a “flat” look with a gray tone underneath the porcelain. Often after the traditional crowns are placed, the metal edge around the gum line becomes visible, resulting in a dark appearance of the tooth and gums. The all porcelain crowns have no metal so that you get a more translucent, natural look and no metal edges. They can also match the color of your natural teeth more effectively than traditional crowns. The life expectancy for both types of crowns is generally five to twenty years depending on the health of the tooth, and the patient’s home care. Patients with periodontal disease or whose teeth are severely broken down are not ideal candidates for all porcelain crowns and may need crowns with additional metal support. Some teeth that have had root canal treatment may also need traditional crowns. Smile Planning: It’s critical that you take an active role in the smile design and spend time in the decision making and planning of your smile. While all porcelain crowns are a wonderful way to enhance your smile, it is important to have realistic expectations. If several teeth are involved, we may want to have a consultation in order for you to feel comfortable with the final appearance. The consultation includes an evaluation of the present conditions, a visual examination and sometimes x-rays and models. We then take before photographs and use wax on the models to give you an idea of what the final restorations will look like. Preparation of teeth: This appointment will take from one to three hours depending on how many teeth are involved. Dr. Sayeg will remove any old restorations and shape the remaining teeth to fit under the crowns. Several impressions will be taken of the teeth, which will be sent to a professional dental laboratory with a prescription and a detailed plan for how the permanent crowns are to look. The laboratory will then fabricate the crowns within a three-week period. Dr. Sayeg will make temporary crowns to protect the teeth during the interim, but since the teeth have been reduced, they will look a little different until your permanent crowns are delivered. Until your next appointment, you will need to avoid extreme temperatures and any hard or chewy foods. Delivering the crowns: This appointment will take about one to two hours depending on how many teeth are involved. First, we remove the temporaries and place the crowns in with a try-in paste to get a sense of the shade or color. We view the esthetic results, and pay particular attention to the color, shape, and length. Using a full-face mirror, we will have you look at your teeth from different positions, under different lighting, while sitting and standing. If you are not pleased with the result it may be necessary for the lab to make adjustments on the crowns or change the color. This can usually be done in a few days. If you are pleased with the results we will then seat the crowns with permanent cement or bonding agent and polish the crowns. What can I expect after the crowns are delivered? For a week or two you may go through a period of adjustment as you get used to your new teeth that are now a different size and shape. It is also not uncommon to have some slight sensitivity to hot and cold for a short period of time after the crowns are delivered. If several crowns have been placed, Dr. Sayeg may want to see you three to seven days after the delivery of your crowns to check on the appearance, function, and the health of the surrounding gums. It will then be important to continue your routine home care of brushing and flossing daily, as plaque and food can cause decay around the gum line and under your crowns. You will also need to continue with your routine preventative visits two to three times a year. What are porcelain veneers? Porcelain veneers are ultra-thin shells of ceramic, which are bonded to the front of teeth. This virtually painless procedure requires little or no anesthetic, and can be the ideal choice for improving the appearance of your front teeth. Veneers have become increasingly popular over the past 15 years, and hundreds of thousands have been placed to mask discolorations, brighten teeth, and to give smiles shapelier appearances. They are highly resistant to permanent staining from coffee, tea, or even cigarette smoking, and can achieve a tenacious bond to the tooth, resulting in an aesthetically pleasing naturalness that is unsurpassed by other restorative options. Why would you want a porcelain veneer? Porcelain veneers are an excellent alternative to crowns in many situations. They can provide a conservative approach to changing a tooth’s color, size shape, or close small spaces, while still maintaining the natural structure of the tooth. Dental bonding materials have, just recently, given dentists an amazing tool to quickly change the shape and color of teeth. It is typically painless and can give dramatic results in just one brief office visit. Today's high tech bonding material consists of a tooth colored, putty-like resin that is sculpted directly on to the tooth surface that needs correcting. After preparing the tooth, Dr. Sayeg will take this resin and shape and color it to make your smile perfect. Then a high-intensity light causes the resin to harden as hard as your natural tooth. Afterward the new surface is smoothed and polished and looks and feels just like your natural teeth. Bonding can lighten stains, close gaps and change the shape of crooked or misaligned teeth. It can be used on one tooth or even your entire smile. Missing teeth create many dental health problems and can make your smile unattractive – affecting your personal relationships and self-esteem. Each tooth serves a function and supports the one next to it, so once a tooth is missing the remaining teeth will shift or rotate from their normal position. These teeth are vulnerable to gum disease, decay and your bite may even be affected, putting stress on your jaw structure and possibly causing TMJ (temporomandibular dysfunction - a painful and debilitating jaw and teeth problem.) Dr. Sayeg will recommend a bridge, in many cases, to fill in the empty spaces. The Bridge will be composed of crowns that are permanently attached to the teeth on either side of the missing space or spaces. A new tooth or teeth will be sculpted to fit exactly in the missing areas and will be permanently attached to the two anchoring crowns. These will look and feel just like your natural teeth. They will be the same size and shape as your natural teeth and as a result will keep your jaw and bite aligned. The bridge will make sure that other teeth do not shift and take on a greater share of the work of biting and chewing. The ideal time for your child’s first dental visit is when your child’s first (primary) teeth come in, usually between 18 and 24 months. Because dental problems often start early, the dentist will want to carefully examine the development of your child’s mouth so the sooner they have their first visit the better. At this visit problems such as baby bottle tooth decay, teething irritations, gum disease, and prolonged thumb sucking can be diagnosed and the dentist can provide or recommend special preventive care. Preparing for the First Visit Appointments for children should always be scheduled earlier in the day, w hen your child is fresh and alert. You may want to bring your child to your cleaning visit so that they can observe and feel comfortable in the office. What happens on the first visit? Many first visits are just introductory icebreakers to acquaint your child with the dentist and the practice and to gain the child’s trust. Durathins Columnist Tara Parker-Pope answers reader’s questions. May 18, 2004: Page D6 Q: I am wondering if the value of calories eaten equals the value of calories that are exercised off. If I eat 2,000 calories a day and do exercises worth 800 calories, is this the same as eating 1,200 calories and doing no exercise. A: Not quite. In theory, a calorie eaten should be the same as a calorie exercised, but the math doesn’t really work that way. When a person starts exercising, all kinds of compensatory mechanisms kick in to defend our body weight and hold on to as many calories as possible. From an evolutionary standpoint, our bodies were designed to maintain weight when food was scarce and gain weight when food is abundant. So the natural tendency is to increase eating to make up for calories lost to exercise. If you have the willpower to resist those hunger pangs, the body still finds a way to conserve calories. You may get up a little slower in the morning or slouch a little longer in your chair—burning a few less calories here and there to make up for calories lost through exercises. And once you start exercising, you body will become more efficient. The mile you ran last week that burned off 100 calories may burn off only 90 calories the next week. From a practical standpoint it takes an enormous amount of exercise to burn a meaningful number of calories—and very little food to erase it. It would take six days of walking 30 minutes a day to burn the 800 calories in your question. And even if you do manage to burn off 300 calories after 45 intense minutes on the treadmill, it just takes one cookie or two apples to erase those lost calories. Indeed, numerous studies have shown that exercise alone doesn’t help people- women in particular- lose weight. Exercise alone is “not a strong enough intervention in the face of other choices folks can make,” says Joseph E. Donnelly, director of the center for physical activity and weight management at the University of Kansas in Lawrence. What are dental implants? Whether you have full or partial denture or have recently lost a tooth, implants may be an option for you. When patients lose teeth, their ability to chew and speak is reduced, and the bone that once surrounded the tooth begins to shrink. Dental implants are substitutes for natural tooth roots and can be used to anchor artificial teeth or give additional support for dentures and partials. They give these replacement teeth a more stable base and improve the use patients can get out of their dentures, partials or bridgework. Implants are small (usually 4 mm in diameter and 8 to 15 mm in length) and long lasting. They are made of light titanium metal, and some are coated with a bone-like substance (called hydroxyl apatite or HA) that blends with the bone in a process called "Osseo Integration". Who is a candidate for dental implants? Anyone who has lost a tooth may be a candidate for implants. For single tooth lost, patients consider implants in order to avoid grinding down healthy teeth to place a fixed bridge, or to help maintain the bone structure of the jaw. They are also considered when back teeth are lost as an alternative to a removable partial denture. When patients already have removable dentures or partials, they may consider implants in order to be able to eat or speak with less discomfort. In order to place implants, there must be enough bone density to support the implant and the final restoration. We may need to take certain x-rays of the jawbone to determine this, and models will be taken of your mouth to determine if your other teeth will bite on the implant restoration correctly. Patients with chronic or severe medical or psychological conditions may not be candidates for dental implants. How safe are dental implants? Dental implants are considered a safe and effective way to replace missing teeth. They are not a new concept, as ancient Incas and Egyptians experimented with “implanting” carved jade, sapphire and ivory teeth thousands of years ago. Implants have been studied in dentistry for more than 50 years, and over one million implants have been placed in the United States since 1986. The metals and coatings used for dental implants adapt very well to the bone, allow the bone to grow alongside the implant, and are well-tolerated by the body. The incidences of bone rejection, or allergies to the implant are less than one-tenth of one percent. How are implants placed? Implant treatment is done in 3 stages. The first state is surgical placement of the actual implant. Implants are placed surgically in an implant surgeon’s office, under local anesthetic. Implant surgery is not difficult, and is usually no more inconvenient than having a tooth extracted. Patients can expect a one-day to one-week recovery period, depending on the type and number of implants placed and the condition of their oral health before surgery. Three to six months later, the healed implant is exposed to allow the gum tissue to heal properly. Lastly, the implant is built-up with a crown. The entire process takes from six to nine months. How do I take care of my implants? Maintaining proper oral hygiene is even more critically important for implants than it is for natural teeth. Poor patient home care will lead to infection and possible loss of the implant and restoration. Implant patients need to understand that oral implants are more likely to become infected than hip or knee implants, for example, because the mouth is constantly exposed to plaque and food debris, which break down gums, teeth and bone. Patients must learn to effectively brush and floss three times a day and maintaining regular contact with the implant and restorative dentist is essential. Smoking, chewing tobacco, gum and hard foods, grinding teeth, and keeping dentures in the mouth at night should be avoided to keep implants healthy. How long will implants last? Most implants have a 15-year, 90 percent survival rate. The success of dental implants depends on many factors, including the patient’s bone type and amount, the patient’s medical condition, the bite relationship of the remaining teeth, cosmetic concern, personal expectation, the training and experience of the implant dentist, the number and type of implants chosen by the implant dentist, and the quality of the patient’s home care skills once the implants are in place. How much do implants cost? Implants are an investment. Fees vary according to the number of implants required and the design recommended by the dentist. Implant therapy involves the cost of the surgery required to insert the implant and the cost of the artificial teeth to be placed onto them. Depending on the type of implant used, one implant could range from $1,400 to $2,000 for the surgical fee and $1000- $1,500 for the restoration. Sometimes other surgical procedures or restorations may also be necessary. Will my insurance cover implant therapy? When investigating the availability of insurance coverage for dental implants, keep in mind that implant treatment is twofold. Sometimes a medical plan will allow the surgery portion of an implant procedure, and your dental insurance might cover some of the restoration cost. A patient’s medical plan may provide coverage if it can be established that the inability to chew is affecting general health. Although pressures are being brought to include more implant coverage in dental benefit plans, the current reality is that few have such provisions. Often, implant therapy is considered elective and many insurance carriers will not cover it.
